The environmental systems reviewer was required, under the mission protocol documented in E1, to sign off before the activity window opened. The authoritative mission sign-off record shows the physiologic and operational sign-offs completed, and the environmental sign-off absent, and the activity proceeded regardless. The Engine can demonstrate that a documented three-part gate was satisfied on two of three required elements per the authoritative record. It cannot establish why the third sign-off was never produced or whether its absence had any operational effect.
